4张2×2 彩色身份证照片复印件;

学生许可证 – 原籍国大使馆出具的学生在菲律宾学习资格的证明;

2份申请人原籍国信誉良好的银行出具的支持或证明宣誓书副本，证明他/她有能力支付在菲律宾逗留期间预计产生的旅行、教育、个人和其他费用;

出生证明的认证副本;

良好道德品质证明;

菲律宾大使馆的安全许可;

外国人登记证明（ARC）;

个人履历陈述;

医疗证明和药物测试结果;

申请费（不予退还）;

外交部 （DFA） 和移民局 （BI） 的认证。
